Writing on behalf of a family member about to enter senior year of HS. Good overall physical shape, HS swim team for 4 years, enjoys working out at the gym. Very concerted about current inability to meet average scores for certain parts of the service academy fitness assessment, sit-ups and push-ups in particular. Are these attainable and realistic goals?

Why would you think they are not attainable? They are for the most part met by every entering class. The criteria can be found at CFA instructions. Details, with respect to USMMA are at CFA

You should understand as well that the numbers are not minimums. Take pushups for a male, 75 is the maximum and the time interval is 2 minutes. That would get you a perfect score for pushups. But the reality is 54 is average. Once all the categories are tested, there is a composite score created and that is used for ranking. It is my understanding that a recent “average” composite score is around the 550 to 600 mark.

There are lots of ways to improve on the test quickly. First is technique - go look at the videos for service academy tests or military PT tests. If the exercises are done "strictly" than scores will be lower. 2nd thing is working out for the tests specifically. Abs/Core can improve greatly in very little time.

KP is not known for having rigorous PT standards. You don't have to be in SEAL Candidate shape to get in and they test to the Navy PRT standard while they are there. The other service academies test to standards ABOVE their respective service component.
