SF600

Filling out the SF600 as part of the Appointee Kit. A few questions:

1) Instructions say to initial on indicated items on pages 4 and 5... there are also spaces to initial on page 3. Safe to say I should just do the ones on 4 and 5?

2) Top of the page says "sign each entry". Does this mean I should sign every time I marked the page (yes/no, explanation, etc.), or just when I've written something down?

I just circled yes or no and put the date next to each entry.

I circled, yes or no, initialed where they were asking to "accept", and dated.
