SLE Medical Questionnaire

Is anyone having problems with the medical questionnaire? Every time I click the button to send myself an email of my medical questionnaire I get directed to a page that says the URL no longer exists.
 
Has it worked yet for you? I still am getting redirected to a page that says the URL no longer exists.
 
They said they will email us when it is fixed, so I am not worrying about it yet :)!
 
Ds and I were able to email the medical questionaire to him this morning. It is all printed out and ready to take to the dr.
 
I believe my son filled out medical information on the SLE portal. I thought that is all there is regarding medical. Is there another form for doctor to fill out? Or is this the medical power of attorney everyone is referring to?

There are some words at the top of the medical form page you need to click on to email it to yourself and print it. Then you need to have the doctor sign it and upload it back to USMA in the portal. It was easy to miss. I went back and did it.

Thank you soooo much for sharing!! I'll be looking over his shoulder a little more closely going forward!!
