SLS application question

For those who got accepted into SLS....

On the Privacy Act Statement form, are we (the attendees) supposed to sign and put our SSN number, or are our parents supposed sign it?
 
"Your parent or guardian must sign in the block marked 'SIGNATURE OF PATIENT OR SPONSOR.' " That's straight off the directions page. So yeah, your parent's signature and social security number.
 
oh wow i feel stupid. i read the entire statement page trying to figure out what it was saying. thank you!!
 
Lol don't worry about it...I wondered about that myself for a while before rereading the directions, and then I too felt a little dumb.

No, it isn't both your parent's signature and SSN. I emailed Commillia Oliver with this same question. Her response, and I quote...

"The form requires Parent signature, Student's SSN. The parent approves release of Medical information and the Patient will be you."

And I'm leaning towards Week 1 myself. Am sending in the the forms tomorrow. My top 3 preferences = History, Mock Trial, Computer & Technology.

Anybody think that I have a chance of getting those 3 workshops for week1? It seems most people on here are applying for Week 1, that I'm getting a bit nervous that I won't get those. (But I'd still be happy with whatever I got, since it's the whole experience that is what makes me so ecstatic. I'm so happy to have this opportunity in general)
 
It seems like the engineering workshops have been the most popular...I haven't heard much about history, mock trial, or computer tech
 
So maybe I'll luck out and end up with them...undesirable (hahah) workshops!

But that's helpful to know, still gives me hope that I'll end up with those three.
 
I wondered that for a bit at first, too, before I read over the directions in the beginning. :)

I have a question, though, while we're on the subject. On the physical form, I got my physician's signature and everything but she was telling me I needed another TB shot (A different type than the one I already got), so should I send in my form now with my current vaccination information ('04) or should I wait until I get the new one (Don't know when that will be...)
 
Well that's up to you, I would guess. They said that they won't process any applications until all the forms are received fully completed. (And that they prefer getting all at once..I believe. Not sure on that though.)

I needed an updated Tetanus Booster myself, managed to get it the same visit during my physical. Guess I lucked out there.

Right, that's what they called it, "booster." For the most part I attend my physicals without my parents, but since I'm only 17, they can't give me shots without permissions.

Guess I'll schedule it ASAP!
 
Really? I'm 16, and attended my physical alone, and got my shot on top of it all too, with no problem. Maybe it's different here in NY? (If you're not in this state) Eh, who knows.
She said that it would be sore the next morning, after the booster, and it didn't hurt at all. I know I got the shot... saw the slight bleeding, saw the needle normal before she put it in (didn't watch. After passing out once during a blood drawing, I don't watch what they do with needles, even though I'm not afraid of them) and the needle come into view with the top depressed, meaning it was used....Maybe I'm immune to the pain and soreness, hahah.

But good luck with that SVG, hope you'll get it soon! (And maybe see you at SLS!)
 
I just applied tonight for the SLS. I was just wondering how long it takes to be notified of acceptance or if your are not accepted. any info would be great
 
I can tell you from my son's experience, that last year he applied to SLS on February 1st, and got an acceptance in the mail a couple of days later.
 
I applied December 14 late at night and got a letter saying I was accepted on the 26. It was dated the 15th though, so yours could possibly come even sooner without all the holiday mail slowing the post office down.
 
I applied on Dec 26th, and got mine in early January, the 8th, postmarked the 5th. (But holidays made it longer probably)
