Some unfilled parts in form

I recently had my eye exam and there weren't any issues! However, the doctor didn't fill out "24. PC in mm" (because he wasn't sure what that was— he was a Korean optometrist). Some other sub-parts were left blank, but 95% of the form is complete. Would this be an issue with DoDMERB, even if everything else is normal? Thanks!
 
Our DS optometrist left one field blank by accident, and DODMETS sent it back to the optometrist to correct. Recommend working with your optometrist to make sure it is corrected and returned.
 
To prevent issues, would also recommend getting it filled out correctly and completely. With proper signatures. All fields file out, following directions carefully.

You don’t want to create further delays and/or complications with your medical exams.
 
Go back and get it filled out. DODMERB will give you an AMI if not filled out completely and you'll end up going back anyways. It's a MPC measured from bridge of nose. My optometrist was confused as well and noted their assumption on my form as such. I have no idea what MPC means.

Will do. Do you know what your optometrist did for the measurement? I'm unsure how to explain to my optometrist what information he needs to fill in for that section.
 
I feel like an optometrist should be able to check their medical journals/peers/boards or Google to find, if nothing else. S/He should be figuring it out if S/he is signing their name to it. I suspect they can figure this out, if you explain the importance of it.

Or, an American doctor??
