Stick and Poke tattoo

I have two tiny (both can be covered with one fingertip) "stick and poke" tattoos from a couple months ago, or ones not done deep enough to last permanently. They will fade until gone, one of them has already faded considerably. They are both completely hidden when wearing a swimsuit or regular clothes. Is this something I should put on the body alteration form?
