Surgery Documentation

My son had a procedure perform on his knee last year after football season. the DODMERB application asks to disclose all surgical procedures. Nothing was found in the procedure other than some bruising. His knee has completely recovered.

He has his scheduled DODMERB physical schedule in a couple weeks. Should we take documentation from the surgeon with us to the physical or wait until they request it?

Wait until DoDMERB requests the information before sending it in. You will want to get copies of all medical records from the time of the initial injury through the time he was released to full activities by his surgeon. I would also recommend that you obtain the copies and mail them in yourself, rather than waiting for you physicians office to mail them in.
