Alexander nominated at least 3 people all with LOAs they have turned into offers of Appointment. One or none of those could be 'charged' to Alexander

Could you please expand on this ? If the MOC is not charged against the slate because of the LOAs, then who/where does the appointment gets charged ?

Where a NOM gets issued may not be where the SA charges it. Lots of older threads on that topic.
