I was just wondering how I am suppose to complete this form. The application says "You must be counseled by your Admissions Liaison Officer (ALO) before you can complete this form." Does this mean I am supposed to have an interview or can this be done over the phone/email. Thanks.
 
Contact your ALO. His/her name and contact info are in your portal.

My ALO says there are at least two different interviews, one being the drug and alcohol briefing and the other being a formal assessment.
 
Correct!

I will always meet with the candidate and we will go over the form completely. There is also a "script" I am REQUIRED to brief to the candidate. Once all this is completed, I can then "certify" that the requirements are completed and then the form will be "unlocked" for the candidate to accomplish.

My ALO went over it with me during my interview, but I figure it varies.
