It could be a call from your congressman, an email telling to check your portal, random portal update without an email, or they could send you something in the mail, but I don't think they've done that in a while. Sometimes the Congress critters will notify you first, and then you'll get the update a few days later, but a lot of them do not call. Usually the first thing will be an email saying to check your portal, and then a portal update with your offer. Good luck!

At the time of my DD’s appointment in early December 2018, she had only a presidential nom on file.

She got an email telling her to check her portal.

Could be many different ways.
DD was notified via email and portal update, nothing in US mail.
