My DD is preparing to apply to the VP for a Nom. The online app is pretty straightforward. However, DD is unclear about what to do with her letters of recommendation, SAT/ACT, Transcript, etc.

The web page states:
Due to a large volume of applicants (usually several thousand per academy), the Vice President has authorized the academies to evaluate all applications and rank them according to qualifications. In screening candidates, the academies evaluate an individual's high school or college academic record, results of the ACT and SAT tests, recommendations from individuals who are in a position to judge the applicant's character and academic potential, and physical and medical qualifications. In view of this screening by the academies, it is important that all correspondence subsequent to actual application (e.g., letters of recommendation, high school and college transcripts and ACT/SAT examination results) be forwarded directly to the academy when requested. Do not send this information directly to the Vice President.
It lists a specific address for the USNA: Dean of Adminssions (Code A), USNA...etc I assume the "Code A" in the address will keep these things separate from the USNA application?

Is it also necessary to send the other information (ACT/SAT, HS Transcript, etc) to that address or will they use the information contained in the candidate's USNA application?

Does the "when requested" mean that the SA will request the letters/information or should they be sent to the the address above when the VP's online application if filled out?

Just complete the online application. You don't have to do anything else. Each service academy will select the VP nominations using the information collected in their normal application process.
