1786: Service Address

I am filing my 1786, and do not know what to put for the service address. I have searched the web and asked the SOC; although, both resulted in an insufficient answer. The SOC suggested to use my work address (street, base, state), though the form says to include your Majcom. Is there a different address template?

Last year my son just wrote The base address, squadron & wing. I remember his counselor calling for his MAJCOM, later in the process.
