Do you need to be an enlisted in order to join AFOTS

Not in the Air Force, but I have a 6th Degree Black Belt in "Google-Foo"

No, for all services. OCS/OTS requires a four year college degree & passing a physical but that's about it. Of course, STEM degrees are always preferable (except Marine Corps OCS, which doesn't care what your degree is for). And Air Force OTS is probably the hardest to get into (Army OCS being the easiest - but not "easy").

Generally speaking, going from enlisted ranks to commissioned officer is exceedingly difficult. For all branches of the armed forces. Not impossible, but rare.

Of course the times change your chances, i.e. 1951, 1967, 2004 = better chances.
 
Another problem going from the enlisted side is current manning levels within your AFSC/Rating/MOS. If your “job” is under manned the willingness of personnel management to let you go is closer to NO than yes. OTS/OCS you still need a 4 year degree. It is possible from the enlisted side but the numbers are against you, think low single digits.
