Immunization Form

    Hey everyone,

    I sent in my immunization form about a week and a half ago and there's nothing yet on the portal? How will I know if anything went wrong or they didn't receive it?

    Bring a copy with you to R_Day just in case.

    Do bring a copy to R-Day. It's mandatory. It's written on the first page of the immunization forms (the one you sign right above the small 2-items checklist).

    I'm sure you have copies. If so it doesn't hurt to resend. Same thing happened to my Authoization Card I resent it and a few weeks later I had a green check. And of course take all copies with you on R Day..

    i have a question on the blood typing form. my dd's peditrician found a note in her medical records from when she was first born stating her blood type. but they called her baby girl ourlastname. do you think that's good enough documentation or should she go to a lab and redo this?

    Don't know if you need to redo it, but Red Cross will do it for free if you donate blood. That's how we handled it for ROTC.
