Immunization Record HPV9 missing

Last year's Immunization record had HPV9 on it, but it appears to be missing this year. It only shows Gardasil (HPV) Cervarix (HPV2). Doctors office wants to know where to enter it. Just write it in a blank space? Does anyone have the phone number of the Health Clinic?

There are two HPV vaccines: Gardasil and Cervarix. Depending on which one you had, they should put it in the corresponding space on the Immunization Record.

There could be some incoming Plebes that received the HPV4 (no longer available) so the form is just asking for documentation of an HPV immunization regardless if it is HPV4 or HPV9.
