Yikes for whoever left the EPSQ info in the logging in. What a huge ooopsie. That oughta leave everyone guessin'. Sounds like some serious one hand not washing the other typo kind of things that we all know & love the government for.

Where on the school website can you find Form 86?

Won't be on the website. The form is supposed to come to you from the Department of Naval Science. Call the Academy to ask about the form and get this information first hand.

Thanks to kpbaseballmom for saving many some confusing moments by calling then posting the previous information.

Per the logging in instructions:

U.S. citizens admitted to the Academy must apply for appointment as Midshipman, MMR, USNR. The Department of Naval Science will be mailing you a welcome aboard packet of forms. Follow the instructions carefully! Included will be instructions on completing the Standard Form 86, Questionnaire for National Security Position. This form contains information which will be difficult to collect once you have arrived.
