I just saw I’m pending disqualification. I really need some help and advice on what to do. I already received the scholarship and the nomination.
My insomnia isn’t even that bad and my shin splints are completely normal and entirely bearable. I haven’t used my inhaler in north than 5 years so my asthma shouldn’t be the problem. This is what it says:
Applicant pending Disqualification for D213.00 - Chronic Insomnia within the past year
R259.10 D224.42 - Recurrent periostitis, shin splints, or tibial stress syndrome within the past 12 months, must complete AMI(s) before waiver processing may be considered
R251.27 Please provide copies of all pharmacy records from age - 12
R240.00 Provide copies of all medical and surgical health records regarding history of asthma, reactive airways, bronchospasm, wheezing, and the use of inhaled medications from your 12th
birthday to the present

Not to oversimplify - gather the requested information - exactly what was asked including the pharmacy records and copies of your medical records /surgical records, and send them in as soon as you can completely do so.
You mention not using an inhaler for 5 years, did you have your prescription filled each year, "just in case"? Do you have the unused medications if so? Does your medical record document you had insomnia in the last year? Does your record show you haven't used an inhaler in 5 years? At this point all you can do is provide the details and hope for the best... and line up plans B, C, D in case waiver(s) are not approved for you to activate and use this ROTC scholarship. Good luck to you/ hope that helps, and thank you for your interest and willingness to serve.

Sorry for the late response but after quite a few days of working things out this is what I know. First to answer your questions, my mom did fill out the prescription back in 2022 just in case. Due to this I’m going to be taking a pulmonary function test to send them along with my pharmacy records to prove that I don’t have asthma. I have no idea where the unused medications are but I could probably find them. My medical record does not document anything about insomnia in the past year it’s just something I should have kept my mouth shut on. I know I do actually have late night insomnia however me and my physician are going to be writing in our letters that it my sleep problems occurred in the months leading up to may of 2023 and we’ll state that I was super stressed about finals and AP tests. To make it sound more likely my fault rather than insomnia I’ll say that I used abnormal amounts of caffeine and stayed up late at night playing video games which is certainly not insomnia but just bad sleep hygiene. For the shin splints my physician will also state that I don’t have shin splints it’s just I felt a bit sore from cross country and track and due to that I thought I had shin splints. I’m also getting a letter from an orthopedic surgeon and my track coach that also says I don’t have shin splints. There are some other things like my continually improving running times and muscle mass to make my case air-tight that I don’t have insomnia or shin splints. Is there anything else I should do?
