NROTC nomination to USMA

DS was told by USMA RC that he is eligible for an ROTC nomination to West Point from his Naval ROTC unit and it will greatly help his chances for appointment. RC emailed DS this form to give to his ROTC C.O. Can someone suggest which box to check of these 3? First one is for JROTC, second one is for Army ROTC and the 3rd seems to be JROTC Honor Unit from a different branch of service. The 2nd box seems the most likely to me since it pertains to College ROTC. I suggested he cross out the word ARMY and insert NAVY. The last one is the only one that mentions any other branch of service but is speaking about JROTC I believe.

Your son could also call the RC or USMA Admissions and confirm the correct block/should he annotate to be 100% sure. The JROTC blocks are exactly what they say they are, JROTC HS sources.
