Paperwork after LASIK

I was wondering what paperwork I need to have my doctor fill out in addition to the DD 2870 form (Release of Medical Information) now that I have undergone LASIK surgery.

I apologize if this has been asked previously, I can't find any specific answers on either here or reddit and Fort Rucker has unfortunately been unable to get back to me after calling.

Page 16 is what you need.

A few important points:

1) The most important exam is the one done at least 3 months after the surgery. The data from that exam can be put on the worksheet on page 16.

2) You also need to demonstrate stable refraction at least one month apart. I'm sure you have follow-ups scheduled at this point.

3) If your refraction before surgery was worst than -6 or +4 OR your astigmatism was worse than +/- 3, then you will need a waiver.

4) You will need a corneal topography. As you can see on the worksheet, the eye doctor will need to annotate normal or abnormal. You must also include a colored copy of the scans.

5) If you fall within certain parameters (most people do), then a history of corneal refraction surgery is not disqualifying and therefore won't need a waiver.

The above goes only for AROTC. I am assuming AROTC because you tried to call someone at Mother Rucker.

Yes that is correct, I am a rising MSIII in AROTC. Thanks very much for your response, that answered my question completely. So it looks like now it is just a matter of making sure I get to my follow-ups, checking to make sure I meet all the medical requirements and completing and bringing in the required paperwork 3 months from now. I'll make sure to coordinate between my eye surgeon and PMS on anything else that might need to be completed. This was very helpful info, I appreciate it.

You won’t actually need the paperwork until you get your flight physical.

If you score high enough on the SIFT, you will get the flight physical at camp.

Some kids get done at an Army hospital but is much tougher to arrange.

You are done with your DODMERB exam, correct?
