Permit to Report - Questions

In the "Permit to Report" packet is a life insurance form (Servicemembers' Group Life Insurance). Do most mids opt for this insurance (400,000 at 26.00/month)?

Second:

The US Naval Academy Alumni Associations - do most mids and Parents join this organization?

I have another. When it asks for signatures, are you signing your first, middle, and last names or just your first and last? It says sign your full name in some places on the forms, but I'm not sure if it means first and last or first, middle, and last.
