Pharmacy Cadet In-processing Form

Does anybody happen to have a valid link or file to a .pdf of the Pharmacy Cadet In-processing Form? Although the ""To Bring" on In-processing Day Checklist" section on the portal has the SF600 and SF86 available, there is no link to the Pharmacy form.
 
It's listed on the packing list in the 2025 Handbook, and looking through the past forum posts indicates that others have had access to it, but I'm not sure if I've ever seen it in my portal.
 
I haven’t seen that form on my portal either, although I do see where it is listed on the to bring checklist. There’s no box to check off that it’s put in the manual envelope though, so maybe it’s just for certain people?
 
The Cadet Clinic has removed the Pharmacy form for this year. It is possible that some part of the instructions did not get updated. Can you tell me where you see the reference? I can try and get it corrected to reduce any confusion.

Page 13 of the 2025 Appointee Handbook, the "***HAND-CARRY THESE ITEMS OUTSIDE OF LUGGAGE*** table, line 11, " Pharmacy Cadet In-Processing Form".
 
It also appears on pages 14 and 15 in the 2025 Appointee Handbook in the Example Male and Female In-Processing Checklist, and in the checklist generated for me in the portal's "'To Bring' on In-processing Day Checklist."

I printed my In-processing checklist and the Pharmacy form isn't on this year's checklist to submit. I think what is in the Appointee Book might be an example checklist from prior years and not updated.

The pharmacy form is on your printed checklist? I saw where medications are to be checked off.
 
It's not next to any checkboxes, but is listed as something that we should be able to print off in the "To Bring" part of the portal.
 
Thank you all for the replies. I was able to verify that this text is currently on the checklist that prints out from the portal:

"Note: If you are missing the Standard Form 600 (Cadet Preventative Health Assessment), the Pharmacy Cadet In-processing Form, or the SF86
(Security Clearance Paperwork), you can print new copies from the "To Bring" checklist page on your portal."

I will see to it that the checklist is updated this week to remove the reference to the pharmacy form.

As far as the sample checklists in the Appointee Booklet... Drenns3 is correct -- they were generated before the requirement was removed. You can safely ignore them in the samples.
