Does the blanks in the below screenshot mean that nobody has reviewed the file? I would think that the file would be reviewed in it's entirety, but obviously would not know. Maybe these are updated after the fact?

The evaluator in this situation is the math and English teachers and extra curricular person who gave applicant a letter of recommendation and not someone within the admissions dept. Hence why there is no evaluator for transcript etc
 
DS's CFA part is different from yours

DS also did his first CFA at USNA during CVW. But 4 months later he did CFA for West Point at an army base (which was much better one). So he asked USNA to have his CFA updated. But his earlier CFA also had times/dates though

Not sure why it's different, but then again there are lots of things we are not sure of. DS just saw the check mark and "qualified" and guessed he was good to go.
