My son just started filling out the medical form on line (He has his appointments scheduled next week), but we have to keep stopping and going to get more information. The on line form does not appear to allow one to go forward or back without logging out?
Is there a complete blank form downloadable on this or another site that one could review to get all the information together on prior to putting it on line.
Or am I missing something?
