Number three says "phone number" but then lists an unfamiliar number. Is this box supposed to be my home phone, or could this possibly be my PMS's phone #? Just need to know if I should correct it or not.

Checked my ROTC instructors page and his phone number matches that on the form, he must have put it in there when he registered me.
