Sf-86 Drug Section

I have received an appointment to USNA for the class of 2017.
I accepted and I received my paperwork including the sf-86 form last week.
I have tried Marijuana once and Alcohol twice. I am not proud of these choices but everyone makes mistakes. Experimemtal basis only.

I have come clean on the DODMERB and any other forms that have asked about drug/alcohol use.

I plan on saying yes for the part of the sf-86 that asks about drug use in the last 7 years.

My question is whether I need to give specifics about the use of marijuana/alcohol on the sf-86.

It says on the sf-86 to provide date(s) of treatment and name(s) of providers.
What does that mean?

Is there anything on I day that relates to this?

Don't leave any spots blank on the sf86

Put NA in any blank spots.

If you didn't discuss this with a health care provider for purposes of treatment put NA in that block. Be honest, single use of marijuana isn't going to flag you as a security risk.

No reason to enter a lengthy answer into the provider block, simply NA will do.
 
I'm not sure which version of the SF-86 form they are having you fill out. The one on OPM only asks if you have received treatment (yes or no). If you answer "yes" to that, then you would need to provide the additional information.
