It updated to “your decision is ready”. I was accepted; already knew I was because of a phone call last week. My admissions officer also emailed me a congratulations email.

Yes. The 100% will be replaced with an envelope.

I had neither, and did not receive an email. I was told by my MOC a week ago that it would update today

Portal just updated to "Your Decision is Ready". Already accepted. Note: It took about a week after I was informed by my MoC that I had received an appointment for the portal to upload. There was no email or anything, I just checked the portal.

No phone call from my MOC all week so was unsure what this meant (but I guess it just means that not all MOCs call the student). This morning my portal changed to the gold "your decision is ready" envelope.
