When I was seeing a therapist a year ago, she sent me a form for accommodation for tests and extra time on assignments because of my anxiety. She said that if i wanted to use it I can. However, I never sent it to my counselor and so never used the accommodation. Should I still check yes for this question?
 
I would say yes simply bc of the question. It’s not ‘did you use it’. It’s ‘was it perscribed/recommended’. Which is yes. You can further annotate what you said. It was never forwarded to counselor, or used.

But if you answer the question, as asked, imo it’s a yes, without a doubt.

Thank you for your answer. Another question, so does "504 plan" and "accommodation" mean the same thing? Because on the form for accommodations that my therapist sent me, it did not have the words 504 or IEP on there, it just said to give me extra time on tests and assignments. I was also never formally diagnosed with ADHD or anything.
Just want to make sure. Thanks
 
That’s a good question. I have no idea. I’m not an expert on DODMERB medical stuff by any means. I also don’t know the nuances of how a form for ‘accommodations’ from a therapist, who I see for anxiety, turns into a ‘504 or IEP’. Or if that’s even the process.

504/IEP is a school thing. Seems like you've never received a 504 or an IEP if you never "received" any additional time on tests/assignments at school. I would always double-check though since I'm just a random person on the internet. Easiest way to figure out if you have an IEP/504 is to email your counselor at school!

I agree. An IEP and 504 is a process, not just a recommendation from a therapist. I know all my students who have them by simply clicking on our Student Information System. I've sat in numerous IEP meetings. Also, IEPs are the domain of the Special Education department and 504 plans belong to school counselors.

How do you answer that question? That's up to you but talking to your counselor about this is a good idea.
 
If the wording is exactly like the thread title, then I would also assume No. The therapist didn't prescribe (or necessarily recommend) a 504/IEP, only provided evidence to support a 504/IEP as an option. (based on info provided).

Likewise, the school never prescribed or recommended it, as they likely had zero knowledge of it.
 
I would mark no based on what you said. If the school (teachers, admin, school counselors) have never seen a need based upon your performance in the classroom, then it sounds like your therapist was just letting you know resources that were available to you. We learned the hard way not to make assumptions on DoDMERB or overthink the questions.

I am not sure IEP and 504 accommodations are same; IEPs are for both low and high special Ed. Including gifted.
 
IEPs and 504s are definitely not the same, though I guess you could say they are in the same family. 504 plans allow for accommodations, while following the standard curriculum for that age group. IEPs allow for the establishment of new educational benchmarks tailored to the individual student. This is a super generalized explanation, but hits the main points
 
A 504 is for students who need medical accommodations. My daughter (not applying to any academy) has a heart defect and her 504 provides for air conditioning in her classrooms as she has trouble cooling down, unlimited access to water & bathroom, self-limiting PE class, etc. She has no additional educational needs. An IEP (individual education program) is for a student with specific educational needs that requires specialized instruction or assistance.
