What to bring to Examinations?

Hey guys I have my DODMERB physical soon and I just wanted to be sure of what to bring to the appointment. I know I need to bring my printed and completed packet (I had to redo my entire packet by hand because none of the questionnaire or signatures were registered) to my medical provider but what about my optometrist? Do I have to print an extra form to bring to them? Also my audiologist is in the same place as my medical provider so how will that work? I'm assuming I will have both audio assessment and medical during the same appointment. Also I noticed at the top of the first page theres a box that says OMB approval expires: Nov 30 2009. What does this mean? Is it just outdated or something? I'm a little worried about this because the whole printing process didn't work correctly for me so I'm hoping an outdated form wasn't printed. Thanks in advance for help with clarifying the step of the application!
