It is more or less just for folks who are appointed with a stipulation of clearance on I-day and any injuries that would prevent you from doing BCT successfully. During BCT there will be a few exams(concussion, dental, etc.) as well.

Depends on your standing. Appointees have a limited route to ETPs through admissions and they control that process( Sorry I don't have more info, but for specifics may be able to get it). Once you are a basic/cadet you can route an ETP thru your Chain of Command.

Assuming you are a male... you are pretty close to the average on all of these so I'd imagine you passed the CFA. I'd recommend continuing to work on pullups, pushups, and situps as well as your mile run. These will help with other things at USAFA as well.
