When I was seeing a therapist a year ago, she sent me a form for accommodation for tests and extra time on assignments because of my anxiety. She said that if i wanted to use it I can. However, I never sent it to my counselor and so never used the accommodation. Should I still check yes for this...

Hi, currently applying for ROTC, one of the criteria for the athlete in the SAL criteria is "Membership in Regional/Competitive League" and what exactly does this mean? If I run and competed in cross country at my high school does it count?
