Accessions Screening Questionnaire

Received my appointment yesterday, accepted, and immediately started filling out the various forms provided to me by USNA. However, when I go to click on the accessions questionnaire it immediately tells me I can’t open the file. I know this is a common problem from last year so does anyone know how I can access the form?

Not sure if the systems are similar but we found success with NASS forms using Internet Explorer browser on a PC. DD had no luck with ipad, safari and chrome but eventually got it to work with IE.

Download the Adobe app, then download the Accession screening questionnaire to your computer. You should be able to open the pdf through the adobe app. That’s what we had to do for ‘26
 
Figured it out. Just had to download and then use the Adobe app. Once I complete all of these files (Acessions, Police Check, etc.) what is next?
 
Go “read ahead” and browse the Plebe Summer pages for Class of 2026, which will not be updated with your class’ specifics until the spring. Look through the hamburger menus. In particular, look at the PTR (Permit To Repprt). Most of it does not change from year to year.

If you haven’t gotten this year’s flu shot, get that done. Might be hard to find in May. Review the other immunizations for those that are taken in series and need spacing. COVID - no idea what the policy will be.

If you don’t have a passport, get an application going. You do NOT need one to report in, but you want to be ready for international travel opportunities. It’s much easier to get one while you are still at home than when you are a plebe.
