On the Chronological Record of Medical Care, there is a spot for ECG results and a cardiologist screening. I am assuming that screening will happen at Swab Summer and that we don't need to get that done before mailing in the packet. Am I correct?
 
I don't believe so, but my son just got his wisdom teeth out. Might be best to do it before you get there if you can.
I apologize for the extra question, but to completely clarify, will the dentist questionnaire be filled out at USCGA?
 
My son filled out the top portion all the way down to signature and date. We left the bottom portion blank. It is my understanding that they will have a CGA dentist sign off on that portion. Also it said somewhere if you need any work done like wisdom teeth extractions to get that completed by june 1st.

You are correct. The first page of the two is filled in. Its a survey they have been doing for a couple years no. The pre-paperwork is done so the processing during week 1 moves quickly.

Yes they will have the dental exam there. The only medical paperwork to be signed by your physician is the immunization form.

Speedy recovery for your son!! My son is having his tomorrow and we are very nervous. Do we need to report this to anyone? I think it’s best to get it done now when you have 7 weeks before our R-day.(USMMA)

Are you able to log into DoDMERB and see your status?
Or do you mean you have never initiated and gone through the DoDMERB medical exam process?
Checked your spam folder for any stray emails?
What “package” are you referring to?
Are you on the waitlist?
If you believe you have completed the DoDMERB process, you can contact your assigned DoDMERB tech, found in the left-side menu in Contacts on DoDMERB home page, and ask them if you have anything pending.

There is plenty of help here, please shine a bit more light onto the situation.

I am a physician. Is it permissible for me to fill out my son's immunization form? It's getting uncomfortably close to the deadline and I would like to keep things moving along.

I was just able to bring mine to someone at the front desk and they started filling it out on the spot. I'd do that to avoid unnecessary risk, but otherwise, I'd ask your son's admission officer.
 
Our doctor filled out the immunization form but did not complete the COVID-19 vaccine form. My son received those back in 2021 (not from his MD) and I have his card. I'm wondering if a copy of his vaccination card would suffice? I left a message with the nurse at CGA but wondered if anyone else has run into this?

Of course. Just attach a copy of the EMR (Electronic Medical Record) to the form. That will give them the info they need, and avoid any questions of signing off on your child form. The EMR speaks for itself.

Yes a copy of the vaccination card will suffice. Applicants come from every corner of the US and medial records differ. A completed Covid card will provide the info they need to review your child's vaccination status. It is a government accepted form that the USCG clinic uses as well.

I know there are many questions and unique circumstances, especially for medical; and the forms are not the best. But understand the intent of the forms is to communicate with the medical providers and they can always follow up if they have questions or need more information. The important thing is to get the forms in as best you can so they can open a file on your child and start the in-processing. They will go over some of the information with each patient when conducting the incoming physical and will make additional data requests as needed.

Don't over think this, do the best you can.
