AF Form 4428 (Tattoos)

Why does USAFA need appointees to sign a separate form about tattoos when there was already a form disclosing tattoos and piercings?

Also, how do I go about having my ALO sign the form? My ALO doesn't live close to me and he hasn't been very responsive.

My ALO has not been responsive either regarding this and in general.... I’ve read others having this issue of not getting a response. Seems like a common occurrence. Quite disappointing.

I had my ALO digitally sign the form through email (mine lived about 5 hours away). As for the abundance of paperwork, your application is only the beginning....
