During the health questionnaire necessary for DoDMERB, I marked that I had dizziness. This happens to me occasionally when I stand up too fast; it is not a large issue in my life (I am a Varsity athlete and I have no problems) but I was trying to be extremely thorough because I obviously did not want to look like I had been trying to hide any medical information.

I have never seen a doctor about this dizziness. However, I have briefly spoken to my primary care physician about this during a visit last July for a required sports physical. She was unconcerned.

Now, DoDMERB is requested Additional Medical Information about my dizziness, including: "copies of ALL treatment and/or hospitalization records REGARDING dizziness". I have no medical records regarding my dizziness. I have gotten my primary care physicians notes from our meeting last July and there is no mention of us discussing my dizziness (I assume because it was an extremely minor part of our visit). Should I just tell DoDMERB that, currently, I have no treatment or hospitalization records regarding dizziness? Should I try and get a statement from my doctor saying we discussed it?
