I was just asked for AMI. It said that I can’t use any patient portal information, so does that mean I can’t use MyChart? How does this work?

You need the actual doctor’s notes for the time frame, or condition they want to know about. Just call the doctor’s office and see what they need from you to print and send/ or email the records to you. You will likely have to fill out a form.

It is my understanding that you can not use the portal to share information directly but you should be able to fill out a release of information through your portal and have it sent.

The steps should look something like this(may be dependent on your health care system). From the drop down menu choose contact us - health records - request to release and disclose patient information to someone other than yourself - fill in all the fields completely, who where what, date needed(use today's date and/or Stat or it will likely sit in limbo until someone gets a round to it) - check how you want it sent, US mail, fax etc. You can also request a copy sent to you to ensure it gets sent or for you to send priority mail.
