I received accommodations in high school for anxiety which I was diagnosed with when I was very young. I grew out of it but still received accommodations. I am now applying to USAFA. Will I be asked about this, and do I have to say it if I am asked? If I do say it will I be disqualified?
Go to the DoDMERB site and read each and every item in the left-hand menu links. That will help a great deal. That is your primary source. Lots of great info on the process, the flow of events, FAQs, etc.

This is what I believe is the current standard used for the Q/DQ process:
During the application process, you will answer questions about your medical history. There may be information which will DQ you. It is then up to the Service/SA to initiate a waiver process if they desire. Some things are never waivered, some are frequently waivered, and some are waivered sometimes.
The important thing, you should be able to function and succeed without any accommodations. This includes your standardized tests such as SAT. If you are already functioning well without medication and accommodations, that should be documented by your medical provider to update your current state, and then you will have info to submit if asked for additional info and in any comments on your medical history form. Talk with your parents and the appropriate school officials.
And, “Will I be asked about this, and do I have to say it if I am asked?” - be honest from the get-go. Answer the questions asked with what you have been officially diagnosed with and what your medical record says. If you are serious about USAFA and military life, wiggly thoughts like this are not a good avenue of thought.
