If you are offered appointment, do they typically call or email you? Or does it vary?

Usually, a Fully-Qualified Offer of Appointment appears in the Candidate Information System (CIS or "portal").
Candidates may get an EMail notifying them that there is a message there (on the CIS) or that their status has changed, but I wouldn't count on it.
I would not expect that they would call you.

The school Admissions Office will not call you. If you are a recruited athlete, your Coach will call you. For applicants, with a Senatorial or Congressional NOMs, you may receive a call depending on the practices of the specific office. I am under the impression that SA Admissions Offices inform the NOM source first and extend the courtesy to them to be the first form of notification. There is an agreed amount of time of this head's up notice, and then the school posts to portal at the end of that window. This provides time for a call if the MOC wanted to, but if not, you still get notified electronically.
