Candidate Phase?!

Hi I am applying to USMA. I recently got a packet in the mail with grey envelopes. I have my transcripts with my SAT scores, class rank, etc. and I am planning to send it. But I have a few questions about the candidate portal

1) where it says "request for academic information form not received"
do you just fill in the contact of your counselor? is it that easy?

2)where it says "candidate activities record" do you once again put the information of the contact who will be sending out the transcripts, etc?

3)for the SOEs (school official evaluations) do I put the name of my previous (11th grade) english teacher, math teacher, and chemistry teacher?

****once again thank you. I am grateful for all responses. I want to make sure I fill it out right so that my application can run smoothly.
 
1)Put that in and your counselor/whoever you put contact information for will receive a file to complete (a link through e-mail). There will be questions such as Class Rank (official or estimated), GPA...

2)Send that in to someone who can verify your activities (like a counselor). As soon as you type that in, the contact info will be saved and you will fill in the you CAR with all your ECs, sports, awards and whatnot.

3)Put contact info to any of the teachers of the 3 areas from the 9th, 10th, 11th or 12th grade. They will receive a link by e-mail, in which they will fill in a questionnaire about you. It is consisted of 12ish questions in which the teacher marks from 1 (strongly agree) to 5 (strongly disagree). He will have to explain why he disagrees/strongly disagrees with any statement. There will also be an extra space to put in extra information, if the teacher deems necessary (extra observations).

I will answer these questions to the best of my ability,

1) It should be your counselor, someone who can verify all the information is accurate from your end.

2) For the CAR I put one of my administrators at my high school again for the reason above, so that someone could verify that I actually did what I wrote down.

3) You can use your teachers from last year if you'd like. Whoever you would think could paint the best picture about you. I think it would be a good idea to if your packet would be completed sooner.
