Change of provider preparation question.

Hi all,

DS is a high school sophomore. He has been with one health care provider for most of his life and his records are mostly from there. The rates for that provider are going up more than $500 per month so we are planning to switch to a different (hopefully better) provider. Knowing what you know about what will be needed in the next few years for DODMERB, is there anything you would recommend I do right now before we exit the current health care provider in terms of housekeeping or records etc.

Request your full, formal clinical documentation and if there are ANY diagnoses made, make sure they are shown on there as resolved or get letters saying they are resolved. Same with medication, if there's any on the document it must also say the medication is not being prescribed anymore. You should still be able to request things as needed even if you miss anything.

Ditto this. Make sure everything is resolved. No open ended diagnosis.

Luckily he has had the same doctor the whole time and if I explain this I believe she can notate it how you mentioned.
 
I'd also suggest making sure they include the most up to date list of vaccinations.
 
Just wanted to say thank you for all the advice. We had his visit to address all of this yesterday and his doctor shared that the new provider shares the same system basically and can access everything. Also, she went through his history and checked everything. She checked vaccinations, medications etc and confirmed some things related to medication that was prescribed when wisdom teeth were removed. Overall the visit was super helpful and Im glad we did this prior to making the change. Thanks again everyone, this was really helpful and why the forum exists I would believe.
