Chronological Record of Medical Care

I'm an appointee for the Class of 2021 and had a question about the chronological record of medical care form. So it asks a whole bunch of questions on the right, but on the left their is an empty column titled with "date". Do we put the date we are filling the form out for each row or the date we had that medical treatment for each one or what?
