Civil involvement question

Quick question about whether or not something is a civil involvement. (AFROTC)

So about 8 months ago I got an email from my school saying that I was out of COVID testing compliance. You were supposed to test every week and I missed the first 2 weeks of school. I got an email from my university saying that there is a 3 strike rule and long story short, I only had 2 strikes since I only ever missed 2 COVID tests. I received another email that I had been referred to the office of student conduct at my university, but they never contacted me or told me anything. I didn’t report this to cadre because quite frankly I would never consider this a civil involvement. The director of student services emailed me the same day saying that I was permitted to miss 3 COVID tests in any given semester, and that I had missed 2 already. She just told me to be careful because 3 missed tests could result in a suspension, but as long as I only missed two tests, the strikes would stay on my account and I would encounter no further issues. In the end, I never mentioned this to cadre because 1. I was permitted to miss 3 tests and I missed 2 (2/3 strikes or a warning)
2. I was told I was being referred to student conduct but I never heard anything after that.

Is this is a civil involvement I should’ve reported or am I just overreacting?
 
I’ll leave it to others more expert in ROTC daily business to address this, but there is a general saying in the military, “Errors of commission are usually better than errors of omission.”

As a leader, I’d rather have people update me on a situation, assure me they are on top of it, and I hear all of this directly from them, before I get an email from someone else, and have to send down the chain of command to find out what’s going on, and find myself wondering why the person chose not to report this as just something going on. It may or may not be a “civil matter,” but now I’ve been caught wrong-footed by my host school contact who sent me this info, because I know zip-squat about it. Do you want to be proactive or reactive, and which best shows your future officer potential?
