I know there is a template for applying for USAFA Congressional Nominations. Is there also a template for USMA? I do not see one. If there is not, should I submit another nomination request for USMA using the same template?

Not sure what you are referring to.

Each Senator and Representative has web pages devoted to their specific process, plus contact info for their staffers. Those processes differ in terms of deadlines, information requested, how submitted, interview/no interview, formats/templates, LOR/no LOR, required attachments, etc.

This is yours to solve by combing their websites, and calling or emailing if you can’t find the answer to your question.
