Contracting and APFT

Two questions about contracting. I was recently on hold because of a medical waiver but now am approved medically. During that time, I passed the APFT but then subsequently failed it. Does passing it once during the year allow me to contract? I am already in my second year of ROTC as a nursing student.

For contracting, do I request to be contracted or do I have to be offered a contract?
