COVID-19 questionnaire

DD received an email today notifying her USMA needs a routine COVID-19 questionnaire completed. It indicates the questionnaire can be found in her DoDMETS profile. DD has logged in to both her DODMETS and DODMERB page but is unable to find the questionnaire. Has anyone been able to find it. Thank you.

How long did it take for this form to be updated? We had conflicting information that everything was completed. When we had contacted the admissions, they said it just takes time to update. No mention of the Covid form. We saw that it still says medically incomplete and found out it was this form. We sent it in today but nobody has responded yet. Hoping it updates in time

Mine updated the same day.

No, I did not have a positive test, and no symptoms. It was just answering the covid questions and submitting it to DoDMERB. Very basic.
