Can anyone add clarification to what exactly the DD Form 2005 is giving permission for? I know it has to do with medical records but the wording is confusing. Thanks for any help with this!

A google search reveals several official sources which can shed light. It’s essentially the form that tells the new military member who will have access to your military medical records for official business. On Day 1 of active duty, it goes into effect. It’s also necessary documentation due coverage under military TRICARE healthcare plan. The SSN is that is the person who will be in the military.

I did a google search and only am more confused. Does it mean that the new service member gives the military permission to pull their past records or gives permission for the military person's new medical records (while serving) to be used?
 
I’ll step back and defer to Mr. Mullen. I read it as the new military person acknowledging the military has the right to review healthcare records outside of DoD in support of providing appropriate healthcare to an individual, among other disclosures of how records will be used.

There are hundreds and hundreds of DD forms which govern the life of military members. Most of these are some form of acknowledgement that your military status and records of all kinds can be accessed for purposes of military readiness.
 
DoDMERB only uses the DD Form 2807-2 (which included that information) and the DD Form 2808.
We don't ask for nor use the DD Form 2005:wiggle:
 
OK, then I would submit you ask them then. If they're requesting it, they should have the details of Who, What, When, Where, Why:wiggle:
