Can anyone add clarification to what exactly the DD Form 2005 is giving permission for? I know it has to do with medical records but the wording is confusing. Thanks for any help with this!
A google search reveals several official sources which can shed light. It’s essentially the form that tells the new military member who will have access to your military medical records for official business. On Day 1 of active duty, it goes into effect. It’s also necessary documentation due coverage under military TRICARE healthcare plan. It's being asked for as part of AFROTC.DoDMERB only uses the DD Form 2807-2 (which included that information) and the DD Form 2808.
We don't ask for nor use the DD Form 2005![]()
