DD Form 93

A few questions I didn't see in the instructions or PTR.

For "Service/Civilian category" what branch do I put or do I put civilian?

Do I put USNA as Duty Station?

Do I fill out the boxes for "military only" on the form? (as opposed to civilian contractors who leave them blank)

I wasn't sure what my status would be.

The DD93 is your emergency data. This info will be used in case you become a casualty off duty, during training, or during operational missions.
