DD3150 Form - Overnight Visit

I have been granted a letter of encouragement allowing me to have an overnight visit at West Point. I have received an email informing me all guests must fill out the DD-3150 form or else the visit will be cancelled and will have to be rescheduled. Where exactly can I find this form, and who should I submit this form to?

“DD” means Department of Defense form. You can google it. The date of the form version will be in the lower left. I only did a cursory search for you using “DD-3150.” Check out several entries to find the latest version.

Search USMA.edu for the form, just in case it’s there.

Your Admissions contact should be able to tell you how they want you to get it to them. You may also want to ask what they want you to use for DoD ID number or use N/A or put something else.
 
It is the proof of COVID19 vaccination...just the original series, not the booster. It is required for entry into DoD buildings and spaces.
This is a relatively new requirement, reach out to your Regional Tech and Regional Admissions Officer.
You will want to have a copy of your COVID19 vaccine card or a picture of it on your phone for your visit.
 
The form is a little intimidating as it makes it sound like you will not have access to WP buildings without a vaccination. A vaccination is required for an overnight visit. For a daily visit, a negative test 72 hrs before arrival is adequate.

DS is doing a daily visit in a couple of weeks. Curious if anyone knows if the negative covid test within 72 hrs is sufficient for entry to the WP sports venues. The athletic website only says you have to have a vax card for entry.
 
I believe you upload the form and any other info through your admissions portal. At least, that is what the visit letter said.
 
You will want to reach out to admissions about the day visit and not needing the DD 1350.
Seriously, we need it for every single visit, every single time we visit.
I think USMA is playing a little loose with the rules in order to accommodate unvaccinated applicants and parents. It is the same for major events like graduation--the 72 hrs in advance negative test--shown at the Visitors Center.

I am not sure I understand what I would need to reach out to admissions for. We still have to return the DD1350 but indicate unvaxed on it, complete a negate covid test 72 hrs prior. These were the instructions provided on the visit confirmation letter received directly from admissions. Again, this is only good for a day visit - no overnight.

You may be asking the wrong people. Please reach out to your Regional Admissions Officer and the Regional Tech. You can call the tech.

We are going for a daily visit next week. OTC test within 72 hours is adequate or must it be a lab test?
 
You can ask admissions or perhaps call the West Point Visitors Center.
