Do I need supplemental info at physical

I had a shoulder dislocation in 2010 during a varsity football game in which I had surgery to reattach the labrum. I have been completely stable since the injury with full range of motion, strength, etc...

I am applying to both the Naval and Air Force academies along with ROTC and I just received my information to schedule an appointment for my DODMERB physical. I read that the shoulder dislocation is a DQ but that a waiver is fairly common if stable for at least a year which mine is.

My question is that should I have my Dr. that performed the surgery and follow up provide me with his notes/paperwork and bring that to the DODMERB physical? Is the shoulder dislocation an automatic DQ, or do they make a determination at the DODMERB physical based upon the information from my Dr?

My son had the same surgery in 2010 on the left shoulder and 2011 on the right shoulder, for the same reason-football. He is fully recovered. While we did request the full medical history on both shoulders as well as a letter from his surgeon in advance, he did not take it with him to his appointment. Once the doctors visit showed up on DODMERB, we called them and spoke with the very helpful staff member handling his case. She provided a fax number for us to fax in the medical records and letter. The record of this submission then showed up within the DODMERB account within a couple of days.
 
Below is the medical code that will be assigned to your past injury. It is an automatic DQ regardless of how long it has been stable. Once you have been officially DQ'd, you can request a Waiver review at each service academy and ROTC.
D225.00 History of any dislocation, subluxation, or instability of the shoulder

My DS is in the same process for the 2nd time. His waiver was denied last year since his surgery was only 6 months prior to R-day. We are hopeful that the waiver for this year will be approved and he'll be a member of the class of 2017!
 
If you are applying for NROTC-MO, you're waiver might not be denied as there is not a hard and fast timeline. The MOI at the university told my DS that as long as he is released by his doctor for physical activity and can participate in all of ROTC he is good to go while we are waiting for the process to play out. Pensacola will back pay the scholarship once his waiver comes through. Not his stipend though, he will be out that. The MOI told us that he actually had a student that was a Junior before his waiver came through.
 
DS's waiver WAS granted for AROTC the first round. DS was appointed to USMA then dislocated shoulder in wrestling tournament. Waiver was NOT granted by USMA for class of 2016 but received an LOA for class of 2017. We are cautiously optimistic that the waiver for this year WILL be approved.
