Hello Everyone!

I was wondering, if the document of Body Alteration needs to be notarized or it is just answering the questions, sign it and then submitted in my portal?
Also, where can I find these documents in the picture? I have been looking everywhere and I can not find them. I downloaded the "NAPS 2023-2024 Student Guide" and could not find anything in there, aside that in the "Checklist for Direct Entry" it says that I will need the documents that are in the picture. If someone could send me a link or anything to find these documents, I would be grateful.

Thank you so much! Another question, for the "Police Record Check" did you guys let the Police send it to Admissions or did you submitted it in the portal? I am asking because I think it would be easier to submitted in the portal; but, the message says admissions prefers to get the documents directly from them, so I do not know what to do.

This will be different for everyone - we called around and found out where we needed to go - our local county government offices. Physically went there, paid the $10, and then scanned it into pdf file and uploaded in the USNA portal. Our police would not fill out the form but did give their own separate form and would not sent it themselves, so we uploaded both, then sent an email to USNA admissions contact letting them know to expect two forms (saved as one pdf file). We much preferred this as we were in control of it getting back to Annapolis.

BTW, the pdf packet for NAPS may require you to upgrade your free adobe reader to open it.

Finally, you need to be enlisted in the Navy Reserves by June 30th and these are the forms you will take to the enlistment office. Call around and find an office that has an officer assigned to it - most don't. You need an officer to give you the oath. An option is bringing your own officer (active or retired) of any branch of the armed services to give the oath. Most Navy enlistment offices are staffed by Petty Officers - they cannot give the oath. For us this has been a hassle find one with an officer and most enlistment offices will not know about the NAPS enlistment process though it is in their manual - they just don't see them during their recruitment careers. The NAPS cover sheet 1-3 has instructions for them and the contact person they can call to help them.
 
Look for your closest Reserve Center - any service branch - they will have full-time staff assigned and usually a few officers. Ditto National Guard Armory or training center.

The Navy, Army and Air Force ROTC units at a nearby college or university has officers assigned.

The JROTC units at high schools typically have retired military on the staff, possibly including a retired officer.

The Military Officers Assn of America (MOAA) has chapters all over the country. Chapters are often run by retired military officers.

Check with VFW, American Legion or AMVETS local organizations for retired officers.
