Documents/Paperwork for I-Day

I would really appreciate it if someone could double check the list of documents needed for I- Day. I am pretty sure I have everything based on Appointee handbook and various emails from USAFA, but I would like to quadruple check.
- Social Security Card
- Birth Certificate
- Photo ID
- Security Clearance
- Service Members Life Insurance
- Record for Emergency Data
- Vaccine Record
- Contact/Glasses Prescription if applicable
- DD 2983

Or, you could develop the list yourself and post it as a service to future classmates, and ask them to quality-check from their own lists.

Is the DD2983 for everyone or just recruited athletes? I don’t see where, in any of my materials, it indicates I need to bring this form.

I dont have it either. I believe it is only needed if you responded "Yes" to the question about prior relationships.

There was an email saying for us to bring at least one of the following: birth certificate or passport. Is it wise to bring both? Or do we only need to bring one?
 
And for that "just in case" moment : make a copy of everything and leave it at home in a folder marked "USAFA Docs". That way if there are any questions - for example, we took a cruise 4 years before DS went to I-Day, they wanted exact dates of the ports of call. We had a copy of the docs and could follow along with the answers when he had to call to get that clarification.
 
My kid just texted us today for my other son's passport number. He is in BCT, guess they are doing some paperwork today.
